Harrogate Floating Support for Families
What is the Service?.
The aim of the service is to provide housing related support to
families in the Harrogate district. Helping them to manage and
remain in their accommodation within the community. You do not need
to be a Leeds Federated tenant to apply for this service. The
Scheme is available to all sections of the community in all
tenures. Support will be offered for up to 2 years depending on
your needs.
Who is the Service for?
The service aims to be as wide ranging and flexible as possible
to allow access for people and their families requiring housing
support. This includes:
Homeless families via Harrogate Borough Council living in
temporary accommodation
People with mental health issues
People at risk of domestic violence
People at risk of offending
Older people with housing support needs
People with drug/alcohol related issues
How can I apply?
As the support is offered on a 'floating' basis, applications will
be considered from the following sources:
Harrogate Borough Council
Voluntary and statutory services in Harrogate Knaresborough and
Ripon area
Housing associations
Mental Health services
Police, Health, Social Services and Education
Self- referrals
Internal referrals (existing Leeds Fed tenants)
What is the referral process?
The referral is received from any of the above
agencies/individuals
An interview is arranged with the potential clients
The support worker identifies support needs through a needs
assessment and conducts a risk assessment
The potential clients are informed of the decision whether or not
they have been successful within 2 days in writing and by
telephone.
Support will be offered for up to 2 years.
The Support Package
Leeds Federated supported Housing will ensure that the correct
level of support is provided. Working with other agencies, if
required.
A 'Support Plan' is agreed between both you and your allocated
support worker which will highlight any areas of support required.
These can include:
Budgeting
Tenancy issues
Promoting independent living skills
Signposting
Encouraging Social wellbeing
Empowerment and confidence building
Learning, Education and Employment support
Health related support
Debt management
Managing responsibilities
Anti-social Behaviour issues
Mental health advice
Substance misuse
We do not offer support with personal care
Support Agreement
When you are offered support you need to sign a Support
Agreement within the first 2 weeks.
The Housing Support worker will provide support where possible and
appropriate. We will work with other agencies already involved with
you, and we will also make referrals to other agencies that can
provide further specialist support you may need.
Leaving the Service
If at any time during your support you decide that you no longer
require assistance, support can be ended. Support will be reviewed
at least 3 monthly.
